The Discharge Follow-up RN provides telephone follow up for the discharge patient, i.e. review of discharge instructions, confirmation of required appointments, answering any questions that the patient/family may have since leaving the hospital, and/or referral to the appropriate person/service, as needed. Permission to receive a courtesy call shall be obtained prior to the patient leaving the hospital. The nurse shall demonstrate accountability for patient satisfaction and service recovery.

Associates in Science required. Must be a Graduate of Registered Nurse program with an active Florida RN license required.  Three years of acute care experience required. Life Support Certificate (BLS, NRP, PALS, ACLS) as per department standard. BLS required.
