Job Description:

The Clinical Coordinator will guide the patient through the primary care health care system by managing standing orders and by assisting the providers in tracking interventions and outcomes. The Clinical Coordinator will assist patients in achieving the best possible health outcomes by providing educational information regarding diagnosis, treatment, community resources and referrals. Utilizes standards, guidelines and protocols for determination of care needs. Coordinates with all members of the primary care health team to ensure optimal utilization of resources and services are delivered. Participates in pre-visit planning and manages standing orders. Focuses on preventive care and wellness.

Qualifications:

Associates degree required.  Florida Licensed Practical Nurse.

 

Experience with outpatient care and/or physician practice environment a plus. Experience in care coordination preferred. Excellent verbal and written communication skills. Bilingual (English and Spanish) required. Minimum Required Experience: 3 years
